Heart
to Heart looks forward to the opportunity to work with you as your family
grows. Please be sure to fill-out and submit the online Home Study request
form so that we can begin the process of completing your home study. To
expedite the home study process please download, fill out, and mail in the
form labeled Adoptive Home Study Supplemental Information. This will help
the professional social worker to complete your home study.
As well, be sure to have a copy of the following items ready and on hand
for the social worker at the time the home study is to be completed:
- 3 Letters of Reference
- 1 Letter of Medical Reference for each adoptive individual
- Birth Certificates for each adoptive individual
- Income tax returns (for the last 3 years)
- Health Insurance Card
- Marriage License
- Divorce Decrees or Custody Orders (As Applicable)
